TMS Success PLLC operates this website. This policy explains what we collect, why, and what you can ask us to do about it.

What we collect

When you fill in a form, take the self-assessment, or book a consultation, we collect what you give us: your name, phone number, email address, the city you are in, and your answers to the questions we ask. If you call us, we may keep a record of the call and a summary of what was discussed.

We also collect ordinary technical information when you browse the site, such as the pages you visit and roughly where you are connecting from.

Why we collect it

To respond to you, to determine whether TMS is likely to be appropriate for you, to verify your insurance benefits if you ask us to, to schedule and confirm appointments, and — only if you have separately agreed to it — to send you educational or promotional messages.

Text messages and email

Consent to receive text messages is never a condition of contacting us or being treated by us. Service messages and promotional messages are two separate permissions and you can give one without the other. You can stop text messages at any time by replying STOP, and stop emails using the unsubscribe link in any email. Message frequency varies and message and data rates may apply.

We do not sell or rent your personal information, and we do not share your contact details with third parties for their own marketing.

Health information

Information you give us about your health — your diagnosis, the medications you have tried, your answers to the self-assessment — is treated as confidential.
